摘 要:目的:探索体外冲击波(ESW)对骨-内置物界面生物力学的影响。方法:将34只日本大白兔的68个股骨随机分成3组:松动组、对照组、冲击组。麻醉下将预先制好的人工股骨柄插入股骨上段骨髓腔内,术后6周对松动组及冲击组的人工股骨柄进行机械松动,冲击组的人工股骨柄在松动后1周开始接受冲击波治疗。各组均于最后一次手术后8周处死,取出股骨标本,进行力学测试。结果:无论最大拔出力,还是最大扭转力,冲击组与松动组均有显著差异(冲击组大于松动组,P<0.05)。最大扭转力冲击组与对照组无显著差异(P>0.05),最大拔出力冲击组大于对照组(P<0.05)。结论:ESW治疗有使松动的人工股骨柄再稳定的倾向。Objective.To study the biomechanical effects of extracorporeal shock wave(ESW) on bone - implant interface. Methods: Prosthetic stems were implanted into 68 proximal femurs of 34 Japanese rabbits and then these animals' limbs were divided randomly into three groups, i.e. loosening group, ESW group and control group. Six weeks after operation, the implanted prosthetic femoral stems in loosening group and ESW group were loosened by outside force, and a week later the femurs in ESW group started to receive ESW treatment. All rabbits were killed eight weeks later since the last operation and the femoral specimens were collected and tested by mechanical apparatus. Results: There were significant differences of maximal extractive force and maximal torsional force between loosening group and ESW group. Maximal extractive force in ESW group was significantly greater than that in control group, but maximal torsional force in ESW group was similar to that in control group. Conclusion: ESW treatment can re - stabilize the loosened prosthetic femoral stern.
