摘  要:目的探讨心血管造影对肺血主要来源于冠状动脉肺动脉瘘的肺动脉闭锁合并室间隔缺损的诊断价值。资料与方法回顾性分析2例肺动脉闭锁合并室间隔缺损及冠状动脉肺动脉瘘的X线平片、超声心动图及心血管造影表现,并进行文献复习。结果2例均经心血管造影诊断并为外科手术证实,分别成功行择期根治术及分流术。造影显示肺循环血供均来自冠状动脉,分别为左冠状动脉及右冠状动脉。结论以冠状动脉肺动脉瘘为肺循环主要供血来源的肺动脉闭锁合并室间隔缺损是一种罕见畸形,术前确诊的“金标准”仍是心血管造影。Objective To evaluate the efficacy of cardioangiography on diagnosis of pulmonary atresia with ventricular septal defect and coronary artery fistula. Materials and Methods 2 cases with pulmonary atresia and coronary artery fistula which were main source of pulmonary circulation, were included in the study, their chest films, echocardiography, cardioangiography were analyzed retrospectively, relevant literatures were also reviewed. Results Cardioangiography showed that the pulmonary circulation originated from a fistula from right coronary artery to the pulmonary in case 1 and left coronary artery in case 2, respectively. Case 1 received corrective operation and case 2 received shunt operation. Conclusion Artery fistula as source of pulmonary circulation in pulmonary atreaia with ventricular septal defect is a rare complex anomaly, eardioangiography is the "gold standard" of preoperative diagnosis.
