L-精氨酸对大隐静脉内皮细胞的保护作用

摘　　要：目的评价肝素化生理盐水和自体全血保存液中加入L-精氨酸后对大隐静脉内皮细胞存活率及分泌一氧化氮功能的影响。方法选取该院连续10例进行择期、单纯冠状动脉旁路移植术的冠心病患者为自身对照研究对象。每例均采用“不接触”技术获取大隐静脉,并切割成环形片段(共26片段)随机分入对照组(未入保存液保存)及实验组。实验组依保存液不同分为:A组(肝素化生理盐水)、B组(肝素化生理盐水+L-精氨酸10mmol/L)、C组(肝素化自体全血)和D组(肝素化自体全血+L-精氨酸10mmol/L)。各实验组分别室温保存30、60和120min。各组大隐静脉片段分别Ho33342、PI双染色及DAF-FMDA染色,利用激光扫描共聚焦显微镜观察各组并分别记数细胞计算存活率及计算缓激肽刺激后一氧化氮产量。结果存活率:各实验组均随时间逐渐降低,各时间点之间及与对照组比较差异均具有显著性。C组、D组在3个时间点分别显著高于A组、B组。D组-C组在60和120min两时间点、B组-A组在120min时间点前者分别显著高于后者。一氧化氮产量:D组在30、60、120min3个时间点均显著高于B组。B、D组在60min分别显著高于A、C组。结论肝素化自体全血保存液对大隐静脉内皮细胞的保护效果优于肝素化生理盐水保存液。保存液中加入L-精氨酸对大隐静脉内皮细胞有一定的保护效果。[Objective] The purpose of our study was to evaluate the influence of heparinized normal saline and autologous heparinized blood with L-Arginine on the viability and the production of nitric oxide of saphenous vein endothelium. [Methods] Saphenous vein segments of 10 isolated CABG-patients were taken with ＂no touch＂ skill. Each vein segment was cut into rings, which were divided into control and protocol. According to the difference of solutions, the protocols were divided into 4 groups： A. heparinized normal saline, B. heparinized normal saline and L-Arginine, C. autologous heparinized blood, D. autologous heparinized blood and L-Arginine. They were incubated at room temperature for 30, 60 and 120 min respectively. Vein segments were incubated with Ho 33342 and PI, or DAF-FM DA after stored in one of the solutions. The viability and the production of nitric oxide of endothelial cells was assessed with laser scan confocal microscope respectively. [Results] Viability： Group A, B, C and D all decreased significantly with time respectively. Group C and D were significantly higher than group A and B, at 3 time points. Group D, at 60 and 120 min, and group B, at 120 min, were significantly higher than group C and A respectively. Production of nitric oxide Group D was significantly higher than group B, at all 3 time points. Group B and D were significantly higher than group A and C, at 60 rain. [Conclusions] Autologous heparinized blood is better than heparinized normal saline for preserving the endothelial cell of saphenous vein. The solutions with L-arginine can improve the preservation of saphenous vein endothelium.
