高原慢性肺心病患者红细胞及血浆镁浓度与心肺功能检查指标的关系

摘　　要：用咔玛琪染料结合法对２４例高原慢性肺心病患者及２８例健康人作了红细胞镁（ＭｇＲＣ）和血浆镁（ＭｇＰＬ）浓度及其与心肺检查关系的研究。结果：①肺心病组ＭｇＲＣ（２．５７±０．５８ｍｍｏｌ／Ｌ）及ＭｇＰＬ（０．８０±０．１５ｍｍｏｌ／Ｌ）明显低于健康组（分别为３．４１±０．４９ｍｍｏｌ／Ｌ、０．９６±０．１１ｍｍｏｌ／Ｌ），Ｐ均＜０．０１。ＭｇＲＣ与ＭｇＰＬ之间无相关。②ＭｇＲＣ与ＦＥＶ１．０％明显相关（ｒ＝０．６７６，Ｐ＜０．００１），ＭｇＰＬ与ＰＥＶ１．０％无相关。③右室肥厚组与右室肥厚伴心律失常组之间ＭｇＲＣ差异明显（Ｐ＜０．０５），ＭｇＰＬ无差异。④ＭｇＲＣ与右室内径（ＲＶＩＤｄ）相关明显（ｒ＝０．４５１，Ｐ＜０．０５），与右室流出道（ＲＶＯＴｄ）及平均肺动脉压（ＭＰＡＰ）相关极明显（ｒ分别＝０．５２５、０．５５３，Ｐ均＜０．０１），ＭｇＰＬ与ＲＶＩＤｄ、ＲＶＯＴＤ和ＭＰＡＰ无相关。提示。Theis study evaluated red cell and plasma Mg(MgRC and MgPL) levels as well as their correlation with cardiopulmonary findings in 24 patients with high altitude chronic cor pulmonale and 28 normal individuals.MgPL and MgRC concentrations in Nls were found to be 0 96±0 11mmol/L.The results showed as follows:①In the 24 patients studied,MgRC and MgPL levels were 2 57±0 58 mmol/L and 0 80±0 15mmol/L,respectively,which were significantly lower than those in normal controls (3 41±0 49mmol/L and 0 96±0 11mmol/L respectively,all P <0 01);no correlation existed between MgRC and MgPL.②There was no significant correlation between MgPl and FEV1 0%( r =0 15, P >0 05),however,significant correlation was found between MgRC and FEV1 0%( r =0 676, P <0 001).③There was a significant difference between decreased MgRC levels in patients with and without ECG arrhythmias( P <0 05);no difference in MGPL was seen.④There was a significant correlation between MgRC level and right ventricular internal diameter in diastole(RVIDd)( r = 0 451, P <0 05),and a very significant correlattion between MgRC level and right ventricular outflow tract in diastole(RVOTd)( r = 0 525, P <0 01)and mean pulmonary artery pressure(MPAP)( r = 0 553, P <0 01);no correlation existed between Mg PL level and RVIDd,RVOTd and mPAP.It is suggested that some relationship exists between the MgRC concentration and cardiopulmonary functional impairment.
