摘 要:目的探讨老年2型糖尿病人骨密度变化及其相关的影响因素.方法QCT测量511例正常成人、48例老年2型糖尿病人和81例老年对照组腰椎松质骨密度,对比糖尿病组与对照组的骨密度,并分析其与绝经年限、糖化血红蛋白(HbA1 c)、胰岛素、C肽、性激素、骨钙素(OC)、尿脱氧吡啶啉(D-pyd)及糖尿病病程的关系.结果(1)老年2型糖尿病男性组骨密度为(75.25±26.76)mg/cm3,女性组为(51.37±21.27)mg/cm3,与对照组比较骨密度稍高,但无统计学意义(P>0.05);(2)老年糖尿病组和对照组女性其骨密度及雌激素水平低于男性组,D-pyd高于男性组.结论老年2型糖尿病人与正常老年人相比骨密度无差异,其骨密度的改变可能与正常老年人一样是年龄增加的结果;老年女性骨密度较男性低,D-pyd较男性高,这可能是其雌激素水平降低的结果.Objective To investigate the change of bone mineral density in aged patients with type 2 diabetes mellitus and to analyze the possible related factors. Methods 511 normal subjects, 48 type 2 diabetics and 81 normal controls aged 65 years or older were selected. The trabecula bone density of the subjects and patients was measured by QCT at lumbar spines. The BMDs of the patients were compared with those of the control group. The correlations of BMD with age, BMI, the time of exercise and sunlight, duration, duration of menopause, HbAIC, insulin, C - peptide, osteocalcin (OC) and deoxypyridinoline ( D - Pyd) were analyzed. Resuits ( 1 ) The BMDs of aged male and female with type 2 diabetes mellitus were (75.25 ± 26. 76) mg/cm^3 and (51.37 ± 21.27) mg/cm^3 respectively, which had no significant difference, compared with those of agematched and BMI -matched control group ( P 〉 0. 05 ) ; (2) In aged women with type 2 diabetes mellitus and the aged control group, the BMD and the level of estrogen decreased and the level of D - pyd increased, compared with those of the aged men with type 2 diabetes mellitus. Conclusion The BMD of aged patients with type 2 diabetes mellitus is not different from that of the aged control group. The change of BMD may be a result of aging. The BMD was lower and D - pyd was higher in aged female than those in male and the possible reason is the decrease of estrogen level.
