摘 要:目的探讨流式细胞术(FCM)DNA倍体分析对恶性胸腔积液及腹水的诊断价值。方法以57例患者的胸水或腹水作流式细胞术DNA倍体分析和脱落细胞学检测,比较二者之间的敏感性和特异性。结果FCM、脱落细胞学检测在良恶性胸腔积液或腹水中的阳性率分别为6.67%,62.96%;3.33%,48.15%。二种方法均有非常显著性差异(P<0.01)。FCM、脱落细胞学检测在恶性胸腔积液或腹水中敏感性分别为62.96%,48.15%,二者无显著性差异(P>0.05)。特异性分别为93.33%,96.67%,二者无显著性差异(P>0.05)。结论流式细胞术分析胸腔积液及腹水细胞的DNA异倍体,对于恶性肿瘤的细胞学诊断有重大意义。Objective To explore the value of DNA ploidy analysis by flow cytometry in diagnosis of malignant pleural effusion and ascitic cells. Methods Flow cytometry (FCM) and exfoliative cytologic examination were used to detect 57 patients with peural effusion and ascites (malignant 27, benign 30 respectively) and compared their sensitivity and specificity. Results The positive rate in benign and malignant pleural effusion and the ascites was 6.67%, 62.92% ; 3.33%, 48.15% with FCM and exfoliative cytologic examination respectivdy. There was very significant difference between benign and malignant pleural effusion and the ascites (P 〈 0.01). FCM and exfoliative cytologic examination's sensitivity was 62.96%, 48.15% and their specificity was 93.33 %, 96.67 % in malignant pleural effusion and ascites. There was no significant difference ( P 〉 0.05). Conclusion The analysis of the DNA aneuploid in the pleural fluid and the ascites by flow cytometry provided the valuable cytodiagnosis for the malignant tumors.
