摘 要:目的探讨T细胞共刺激分子CD54、CD40和B7-1分子在自身免疫性甲状腺疾病(AITD)中的作用。方法采用免疫组化法观察48例桥本甲状腺炎(HT)、51例Graves病(GD)和50例非毒性结节性甲状腺肿(NTG)的甲状腺组织中CD54、CD40和B7-1的表达,应用Mias99图像分析系统进行定量分析。结果①3种T细胞共刺激分子CD54、CD40和B7-1在HT患者的甲状腺组织中呈阳性表达,强阳性染色区域主要分布在甲状腺滤泡细胞胞膜和细胞质;在GD患者的甲状腺组织中大部分呈弱阳性表达,少数病例在浸润的淋巴细胞区呈强阳性表达;在NTG的甲状腺组织中呈阴性表达。②CD54、CD40和B7-1的阳性颗粒面积、平均吸光度及积分吸光度值比较:HT组明显高于GD组,而GD组明显高于NTG组,3组组间差异均有极显著性意义(均P<0.01)。结论T细胞共刺激分子CD54、CD40和B7-1的异常表达可能与AITD的发病有关。Objective To investigate the expression of T cell costimulatory molecules (CD54, CD40 and B7-1) in the thyroid tissue of autoimmune thyroid disease (AITD) and its contribution to the pathogenesis. Methods The thyroid tissue specimens of 48 cases of Hashimoto's thyroiditis (HT), 51 cases of Graves' disease (GD) and 50 cases of nontoxic goiter (TG) were selected. The expression of CD54, CD40 and B7-1 was detected by immunohistochemical method and analyzed by Mias99 pathological image system. Results ① The strong positive staining of CD54, CD40 and B7-1 in all the cases of HT mainly distributed on the cell surface and in cytoplasm; ② The weak positive staining of CD54, CD40 and B7-1 in all the cases of GD mainly distributed on the cell surface and in cytoplasm, but some infiltrating lymphocytes showed stronger positive staining; ③ No positive staining was observed in any part of thyroid tissue from NTG; ④ The positive staining area, average light density and total light density of CD54, CD40 and B7-1 in HT thyroid tissue were significantly higher than in GD, while those in GD thyroid tissue were higher than in NTG control group (all P〈0.01). Conclusion The abnormal expression of T cell costimulatory molecules (CD54, CD40 and B7-1) may contribute to the pathogenesis of AITD.
