医用硅橡胶输精管栓堵的临床应用研究

摘　　要：用荷兰医用硅橡胶对１１例有生育力男子做输精管栓堵。术前和术后１、２、４、６、８和１２月均进行了体检、精液分析和抗精子抗体检测。术后１２月经Ｘ光片测得２０阴囊内栓子的平均横径为６．３０±１．４５ｍｍ，另２侧硅橡胶呈细条状务曲在同侧输精客壶腹和近壶腹的输精管。１０例切下的栓堵段中１８侧有栓子存在，见硅橡胶凝结不紧密，有的还有小缝隙，与输精管壁有的部位也紧贴不严。有８例的２２次 子数降至２０×１０＾６／Eleven married healthy and fertile men who requested vasectomy were recruited for vas occlusion by the method of percutaneous intravasal injection of Ovabloc silicone rubber of the Netherlands. Physical examination, semen analysis ami assay of antisperm antibody were performed prior to and at 1 , 2,4.6,8 and 12 months after injection. The mean width of 20 intravasal ph gs was 6. 30 ± 1. 45mm, which was obtained by measurement on X-ray films taken 12 months after injection. The other 2 plugs twisted in the ampulla and adjacent part of vas in two recipients respectively. The blocked segments of vasa were cut off from 10 locipients. 18 plugs were in the vasa bat the cured-in -place rubbery solid formed loosely with tiny Haws and pores, and the adherence of the rubbery solid to the wail of vas was not well enough. Following vas occlusion , the sperm concentration has reduced to under 20106 ml in 8 recipiets' 22 times semen analyses. Azoosperrmia occurred in only recipient in the 2nd month after in lection , but spermatozoa appeared again m his ejaculate in the 4th month after injection. Pathological examinations revealed proliferation of fibrous on the wall of 20 occluded vasa and sperm granule-ma of 13 vasa. Besides the clinical and pathological findings after injec tiori, 55 % of the cases become positive for TAT, 18% positive for SIT, 64% positive for MAR, and in IBT,64% showed positive IgG adherence and 36% showed positive IgA adherence.
