摘 要:目的:探索不同皮神经移植体修复神经缺损的再生效果。方法:新西兰兔27只,随机分为三组。双侧耳大神经各切下1.2cm,耳大神经移植组将其原位缝合桥接缺损,隐神经移植组用隐神经移植体桥接,股外侧皮神经移植组用股外侧皮神经移植体桥接,修复方法采用外膜缝合。术后1、2和4周,采用神经夹捏试验测量神经再生距离,采用Glees神经纤维染色法观察正常和再生神经结构,采用ImageProPlus图像分析软件进行图片分析。结果:术后1周,各实验组再生距离无明显差异;术后2周,隐神经移植组再生距离大于股外侧皮神经移植组;术后4周,隐神经移植组再生距离大于耳大神经移植组,耳大神经移植组大于股外侧皮神经移植组,组间比较差异有显著性意义。术后4周,隐神经移植组再生轴突数量明显大于耳大神经移植组和股外侧皮神经移植组,耳大神经移植组大于股外侧皮神经移植组,并且隐神经移植组轴突通过率大于耳大神经移植组,耳大神经移植组大于股外侧皮神经移植组,组间比较差异有显著性意义。结论:不同的神经移植体结构对自体神经移植再生效果有着显著影响,神经束面积大且神经干截面积小的神经移植体再生效果较好。Objective : To investigate the neuromechanism of the regeneration of nerve defect. Methods : Twenty seven New Zealand rabbits were randomly divided into three groups. In all the animals, 1.2 cm length of the auricularis magnuses were resected on both sides. In auricnlaris magnuses(AM) group, the defects were repaired with the auricularis magnuses. In saphenous nerves(SN) group, 1.2 cm length of saphenous nerve was transplanted to the auricularis magnuse defect and the lateral femoral cutaneous nerve was transplanted to the defect in lateral femoral cutaneous nerves(LFCN) group. The nerve pinch test and Glees staining were preformed 1 week ,2 weeks and 4 weeks after the operation. The number, density and cross sectional area of the regenerated nerve fibers were measured by Image Pro Plus software o Results: The extent of regenerated nerve in SN group is longer than in LFCN group on the 2 weks after the operation. Four weeks after the operation, thedistance in SN group is longer than in AM group, and that AM group is longer than LFCN group. Four weeks after the operation, the number of fibers in SN group is larger than that in LFCN group, and that of AM group is larger than LFCN group ; the fiber pass-through rate in SN group is bigger than in AM group, and that of AM group is bigger than LFCN group. Conclusion: The different neuromechanisms of the nerve donator have different effects on the nerve regeneration, and the nerve donator with larger area of nerve bundle and smaller area of cross sectional area has better result of regeneration.
