视神经管骨折的CT诊断

摘　　要：目的研究视神经管骨折的CT表现。方法回顾性总结35例(35眼)视神经管骨折病例的CT表现。结果35例均表现为视神经管一处或多处的骨质连续性中断、移位、粉碎,继发神经管增宽或变形。其中视神经管线型骨折10例(28.57%),粉碎型骨折8例(22.86%),嵌入型骨折5例(14.29%),凹陷型骨折6例(17.14%),混合型骨折6例(17.14%)。35例中单纯视神经管骨折6例(17.14%),视神经管骨折伴有邻近眶壁或蝶窦骨折29例(82.86%)。结论视神经管骨折容易造成失明,因此外伤失明的患者尽早采用高分辨率CT检查,对临床诊断及选择治疗方案具有指导意义。Objective To study the CT findings of the optic canal fracture. Methods The authors anzlyzed the CT findings of 35 cases with optic canal fractures retrospectively. Results CT showes that the bone of the optic canal were broken, moved or crushed, and the canal became broad or deformed. In 35 cases, 10（28.75% ） cases showed linear fractures ; 8 （22.86 % ） cases with smashing fractures ;5 （ 14.29 % ） cases with embedded fractures; 6（17.14%） cases with depressed fractures; 6 （ 17.14% ） cases with mixed fractures. 6 of 35（ 17.14% ） cases showed simple optic canal fracture, and 29（82.86% ） cases showed fractures of optic canal combined with fractures of adjacent orbital walls or walls of sphenoid sinuses. Conclusions Fractures of optic canal led to visual loss so easily that patients with visual loss after injury should be examined by high resolution CT as early as possible in order to gain information for diagnosis and therapy.
