摘  要:目的:回顾文献分析评价卡维地洛治疗轻、中度原发性高血压的有效性和安全性。方法:以拉贝洛尔为对照药,运用Meta分析方法,对8个采用随机盲法阳性药平行对照的独立临床试验结果进行定量合成分析。结果:卡维地洛组降压有效率略优于拉贝洛尔组,合并效应量OR为1.58,其95%CI为1.18~2.14,P<0.01。剂量效应关系中,两组治疗的差别无统计学意义,合并效应量OR为1.42,其95%CI为0.98~2.08,P>0.05。在降低心率上,两组在试验前后差别均有统计学意义,但是组间比较缺乏有效数据不能比较,需要进一步研究。两组间不良反应发生率无显著差异(P>0.05)。卡维地洛组主要不良反应为头晕、头痛、嗜睡、乏力。结论:卡维地洛对轻、中度原发性高血压降压效果显著,是一种安全有效的抗高血压药物。Objective: To evaluate the efficacy and safety of carvedilol for patients with mild to moderate essential hypertension. Methods: Eight randomized, double blind positive control clinical trials of carvedilol were quantitatively evaluated using the meta-analysis. Labetalol was used as an active control. Results:Carvidilol was mildly superior to labetalol in the reduction of blood pressure, which gave a pooled OR of 1.58 (95%CI:1.18 -2.14) in the efficacy setting (P 〈0.01).The pooled OR of two treatment groups showed 1.42 (95 % CI : 0. 98 - 2.08 ) in the dose-effect setting ; the P value had no statistical difference (P 〉 0. 05). Carvidilol and labetalol significantly reduced the heart rate. Although the P value of the heart rate clown showed a statistical difference pre and post treatment within each regimen, it reached no conclusion due to lack of sufficient data compared between two regimens. No significant difference of adverse event incidences between two regimens was found (P 〉 0. 05). The common adverse reactions of carvedilol included dizziness, headache, somnolence and tiredness. Conclusion:Carvedilol was effect and safe for the patients with mild to moderate essential hypertension.
