摘　　要：目的:探讨埃索美拉唑与奥美拉唑治疗十二指肠溃疡的疗效对比。方法:EAC组:埃索美拉唑20 mg +阿莫西林1．0 mg+克拉霉素500 mg,OAC组:奥美拉唑20 mg+阿莫西林1．0 mg+克拉霉素500 mg,观察治疗前后两组病人的胃肠道症状变化,以及治疗结束后4周溃疡愈合情况和Hp根除情况。溃疡的愈合情况通过胃镜检查来评价,Hp的状况通过尿素酶呼气试验(14C-UBT)检测。结果:EAC组的Hp根除率为90％,95％可信区间(95%CD为73％-98％。OAC组的Hp根除率为80％,95％可信区间(95％CI)为61％-92％。EAC组患者上腹痛、反酸、烧心等症状的缓解均优于OAC组,差异具有统计学意义。结论:EAC组与OAC组在溃疡的愈合及根除率方面具有相同疗效。EAC组对胃肠道症状(上腹痛、反酸、烧心)的缓解明显优于OAC组。Objective： Proton pump inhibitor （PPI）-based triple therapy is recognized ad the best therapeutics on he Helieobaeter pylori, associated peptic ulcer. Esomeprazole is the newest PPIs. The purpose of this study was to compare the efficacy of esomeprazole or omeprazole at eradicating of H. pylori the eieatrisation of ulcer and the control of clinic symptoms. Methods： A total of 60 H. pylori-positive patients with acute duodenal ulcer （DU） received twice daily treatment with esomeprazile 20 mg （n =30） or omeprazole 20 mg （n =30） in combination with amoxieilin 1 g and elarithromyein 500 mg for 1 week. After completion of therapy 4 weeks later, we compared the ulcer healing, Hp status and the clinical symptom between two groups. Ulcer healing was assessed by endoscopy, and Hp status was assessed by 14C-ure breath testings. Results： The Hp eradication rates（95%CI）were： EACg90% （73%-98%）; OAC80% （61%-92%）. The esomeprazole-based triple therapy can better control gastric symptoms （upper abdominal pain, heartburn, acid reflux） than omeprazole-based triple therapy, and the data has statistical correlation. Conclusions： Esomeprazole-based triple therapy has the same effect with omeprazole-based triple therapy for ulcer healing and Hp eradication in patients with DU disease,and esomeprazole-based triple therapy has a more efficacy for gastric symptoms （upper abdominal pain, heartburn, acid reflux） control.
