摘 要:目的探讨紫癜性肾炎患儿的体液免疫状态及其发病机制。方法对39例紫癜性肾炎患儿及30例健康儿童的血清采用全自动生化分析仪进行IgG、IgA、IgM、C3、C4检测;紫癜性肾炎患儿另行肾脏病理检查与诊断。比较紫癜性肾炎患儿与健康儿童血清免疫球蛋白及补体的差异性,以及紫癜性肾炎患儿肾脏损害与免疫紊乱的相关性。结果紫癜性肾炎患儿血清IgG、IgA水平明显高于健康儿童,而IgM、C3、C4低于健康儿童(P<0.05或0.01)。紫癜性肾炎患儿病理级别间血清免疫球蛋白及补体差异均无显著性(P>0.05)。结论紫癜性肾炎患儿存在严重的免疫功能紊乱,肾脏损害程度似与免疫紊乱无直接相关性。Objective To explore the humoral immunity state and pathogenesis in children with Henoch-Schonlein purpura nephritis (HSPN). Methods Serum IgG, IgA, IgM, C3 and CA levels were detected in 39 children with HSPN and 30 healthy ones using full-automatic biochemistry analyzer. Results Serum IgG and IgA levels of patients were significantly higher, while IgM, C3 and CA lower compared to controls(P〈0.05 or 0.01). There were no significant differences in immunoglobulin and complements as for pathological grades(P〈0.05). Conclusion There was severe disturbance of immunologic function in children with HSPN, and degrees of renal damage may not be directly related to immunologic disturbance.
