儿童苯巴比妥血药浓度与剂量疗效关系及临床意义

摘　　要：荧光偏振免疫分析法（ＴＤＸ仪）测定儿童苯巴比妥血药浓度３０８例，采用Ｆｏｘｂａｓｅ＋建立数据库辅助分析，结果表明不同年龄苯巴比妥的血药浓度与剂量比有一定差异；抗癫痫治疗浓度不必过份强调低限：苯巴比妥体内过程的个体差异较大，通过治疗药物监测有助于个体化给药、鉴别诊断和了解病人的依从性，以更好地发挥抗癫痫药的作用。With the measement of serum phenobarbital concentrations in 308 children by using fluorescence polarization immunoassay and assistant analysis of data base by Foxbase system, the results. showed: there was a certain difference in concentration level/dose ratios(C/D) of phenobarbital in serum of various age groups; some patiente would achieve the desired therapeutic effect at suboptimal serum concentration and it was not necessarily to be controlled in the antiepileptic therapeutic concentration by adjustment of drug regimen; the individual difference in the disposition of phenobarbital within an individual patient was big; therapeutic drug monitoring was helpful to individualization of drug therapy, differential diagnosis and comprehension of patients’compliance with the prescribed drug regimen to give a more rational basis for optimizing drug therapy.
