正常大脑镰与纵裂池出血的CT鉴别诊断

摘  要:目的鉴别正常大脑镰与纵裂池出血的CT表现,以便提高纵裂池出血的诊断准确性,减少漏诊或误诊。方法收集经CT检查确诊为纵裂池出血35例和正常对照者80例,均行轴位CT扫描,将大脑镰与纵裂池以胼胝体和侧脑室为界分为上、前、后3部分进行对比分析。结果正常大脑镰表现为细线状高密度影,纵裂池出血则表现为呈一较正常大脑镰粗的高密度影,边缘模糊,少数呈高密度“之”字征或“Y”字征,正常镰旁脑脊液间隙消失,天幕切迹缘增宽。结论CT对正常大脑镰与纵裂池出血的鉴别诊断有重要价值。Objective To differentiate the CT findings of the normal cerebral falx (NCF) from those of the hemorrhage of the interhemispheric fissure cistern(HIFC) so as to elevate the diagnostic accuracy of HIFC and to reduce its leak diagnosis or misdiagnosis. Methods Thirty - five patients with HIFC and eighty healthy subjects (taken as controls) underwent axial CT scans of the head. CT findings of all patients with HIFC were analyzed and compared with those of the controls. Based on the limits of corpus callosum and lateral ventricle, the cerebral falx and the interhemispberic fissure cistern were divided into superior, anterior and posterior part. Results On CT images, NCF presented linear hyperdense shadow while HIFC presented hyperdense shadow with more thickened than that of NCF, whose margin was hazy, few of them presented as hyperdense"Y" word sign associated with the disappearance of normal cerebrospinal fluid space arround the falx and the widening of tentorial incisure rim. Conclusion CT plays an important role in the differentiation between NCF and HIFC.
