摘  要:目的初步探讨主动脉瓣狭窄伴左心室收缩功能不全患者主动脉瓣口面积估测方法。方法连续方程测量40例左心室收缩功能减退合并主动脉瓣狭窄患者主动脉瓣口面积(aortic valvue orifice area,AVA);由传统伯努利方程(ΔP=4v2)算出主动脉瓣口跨瓣压差(ΔP);用Simpson容积描记法测量左心室射血分数(ejection fraction,EF),计算EF与主动脉瓣前向血流流速(V)比值即EF速度比值:EFVR(EFVR=EF/V),直线回归分析AVA与ΔP、E-FVR之间的相关性。结果对主动脉瓣狭窄伴左心室收缩功能不全患者,用EFVR估测AVA较传统的ΔP法更准确(r=0.903 8比r=-0.679 6)结论EFVR能准确估测伴有心功能不全的主动脉瓣狭窄患者主动脉瓣狭窄程度及瓣口面积。Objective: To investigate a new variable for measuring area of aortic stenosis in patients with left systolic dysfunction. Methods Aortic valvular area(AVA) was measured by consecutive quation, the peak of aortie valvular(V) and transvalvular pressure gredient(△P) was measured by bertouli quation, the ratio of EF to V was expressed with EFVR. Result There were more signifieant correlation between EFVR and AVA than △P(r =0.903 8 vs r= -0.679 6). Conclusion EFVR is an effective variable to evaluate area of aortie stenosis in patients with left systolie dysfunetion.
