充血性心力衰竭患者E/Ea与严重程度的相关性

摘　　要：目的探讨充血性心力衰竭（CHF）患者的跨二尖瓣舒张早期血流速度峰值（E）与二尖瓣环舒张早期运动速度峰值（Ea）的比值和CHF严重程度的关系。方法用多普勒组织显像测定54例CHF患者和29例正常人的Ea，用脉冲多普勒测定E，分析CHF患者E／Ea的变化及其与CHF严重程度的关系。结果①CHF患者的E／Ea高于正常组（14．01±4．53vs8．41±2．17，P〈0．01）。②CHF患者中，心功能Ⅲ～Ⅳ级的E／Ea值高于心功能Ⅱ级的E／Ea值（16．41±5．42 vs 12．34±2．88，P〈0．01）；其中左室射血分数（LVEF）≥50％的CHF患者，心功能Ⅲ～Ⅳ级的E／Ea值也高于心功能Ⅱ级的E／Ea值（16．27±4．96 vs 12．05±3．00，P〈0．01）。③脑利钠肽（BNP）≥1000pg／ml的CHF患者，其E／Ea值高于BNP〈1000pg／ml的CHF患者（16．63±6．45vs13．15±3．00，P〈0．01）。④LVEF〈50％与LVEF≥50％的CHF患者比较，E／Ea值差异无统计学意义（15．06±5．09vs13．41±4．16，P〉0．05）。⑤LVEF〈50％的CHF患者，E／Ea值与LVEF无相关性（r=0．06，P〉0．05）。结论CHF惠者的E／Ea值升高，其数值能够反映CHF的严重程度。对LVEF正常者，E／Ea升高能提示舒张性心力衰竭的严重程度。Objective To evaluate the correlation between the ratio of mitral peak velocity of early filling（E） to early diastolicmitral annular velocity（Ea） and severity in patients with congestive heary failure. Methods Mitral peak velocity of early filling was detected by Doppler tissue imaging and early diastolic mitral annular velocity by Doppler flow imaging. Results ①E/Ea ratios in patients with congestive heart failure were higher than those in the controls（14.01 ± 4.53 vs 8. 41 ± 2.17,P〈0.01）. ②Patients in New York Heart Association（NYHA）functional class Ⅲ or Ⅳ had a higher E/Ea ratio than those in class Ⅱ （16.41 ± 5.42 vs 12.34 ± 2.88, P〈0.01） ,including those with left ventricle ejection fraction≥50% （16.27 ± 4, 96 vs 12, 05 ± 3, 00, P〈0.01）, ③E/Ea ratios in patients with BNP≥1 000pg/ml were higher than those in patients with BNP〈1000pg/ml（16.63 ± 6, 45 vs 13, 15±3.00, P〈0.01）, ④ There were no significant differences between E/Ea ratios in patients with LVEF〈50% and those in patients with LVEF≥50% （15.06 ± 5.09 vs 13.41 ± 4.16, P〉0. 05）.⑤ There had no exist correlation between E/Ea ratio and LVEF in patients with LVEF〈50 % （r = 0.06, P〉0.05）. Conclusion The ratio of transmitral E velocity to Ea in patients with congestive heart failure can be used to estimate severity of CHF, even in patients with normal LVEF. Increased E/Ea ratios in patients with normal LVEF may indicate diastolic cardiac insufficiency.
