摘　　要：目的评价重组乙型肝炎(乙肝)疫苗(酵母)(YDV)免疫的保护效果及成本-效益。方法在同龄儿童中设免疫组和未免疫组,对照研究免疫与未免疫儿童组乙肝病毒表面抗原(HBsAg)携带率之差异及其免疫的保护效果。同时,以本研究免疫组的HBsAg携带率与实施乙肝疫苗免疫策略前山西省同龄组儿童的HBsAg携带率做对比,分析YDV的保护效果,并进行免疫的成本-效益分析。结果免疫组的HBsAg携带率为0.41%,无免疫史组的携带率为3.24%,差异有非常显著的统计学意义;两组对比分析的疫苗保护率为87.35%。实施免疫策略前后同龄组儿童HBsAg携带率比较计算的疫苗保护率为95.00%。分别按全国和山西省的乙肝病毒(HBV)感染本底资料分析免疫的成本-效益表明,每接种1 000人份YDV可分别减少106名和103名HBsAg慢性携带者,27名和26名慢性肝病患者,粗略估算直接的成本-效益之比为1∶27。结论YDV免疫的保护效果良好,并有非常好的成本-效益。Objective To evaluated the immune effect of recombinant yeast-derived Hepatitis B vaccine （YDV） and its cost-effect. Methods The children born in the same year were divided into vaccinated group and no-vaccinated group to observe their carry rate of HBsAg and protective effect. Meanwhile comparative analysis protective effect and its cost-effect use the carry rate of HBsAg in vaccinated group and no-vaccinated group. Results Carry rate of HBsAg in vaccinated group was 0.41, 3.24 was in no-vaccinated group, there was significant difference. According to comparative analysis, protected rate of vaccine was 87.35 %. Compared their carry rate of HBsAg with before vaccination and after vaccination, protective rate of vaccine was 95.00% . Analyzing the cost-effect of HBV according to the background information of Shanxi and all over the country showed that with 1000 doses vaccinating, chronic carrieries could be reduced to 106 and 103 respectively, chronic pa- tients of HBV reduced to 27 and 26 respectively. The ratio of direct cost-effect was 1：27. Conclusion Protective effect of vaccine is better than PDV and also has high cost-effect.
