摘  要:目的探讨脑脊液乳酸脱氢酶(LDH)及β2-微球蛋白(β2-m)检测在鉴别小儿化脓性脑炎与病毒性脑炎中的意义。方法采用速率法检测了45例化脓性脑炎患儿、49例病毒性脑炎患儿及22例对照组小儿的脑脊液LDH浓度;采用放射免疫分析检测上述对象的脑脊液β2-m浓度。结果脑脊液LDH浓度化脓性脑炎组(26.15±12.17)U/L,显著高于病毒性脑炎组(8.76±4.94)U/L(P<0.01);脑脊液β2-m含量化脓性脑炎组(3.56±0.96)μg/L、病毒性脑炎组(2.13±0.44)μg/L,均显著高于对照组(P<0.01),化脓性脑炎组显著高于病毒性脑炎组(P<0.01)。结论脑脊液LDH及β2-m浓度可作为鉴别小儿化脓性脑炎或病毒性脑炎较为快速的诊断指标,从而为临床尽早选择用药提供实验室依据。Objective To explore the significance of LDH and β2-m concentration in CSF in differentiating viral meningitis from purulent meningitis in pediartic patients. Methods Kinetic method was performed to detect the LDH concentration in CSF in 45 patients with purulent meningitis and 49 patients with viral meningitis and 22 healthy children of control group respectively. On the other hand, radioimmunoassay(RIA)was used to detect the concentration of β2-m. Results The concentrations of LDH in CSF in purulent meningitis group(26.15 ± 12.17)U/L were higher than that in viral meningitis group(8.76±4.94)U/L significantly(P〈0.01) ;the levels of β2-m in CSF were higher in both purulent meningitis group( 3.56 ± 0.96)μg/L and viral meningitis group (2.13 ± 0.44 )μg/L than that in normal control group (P〈0. 01). The significance was also apparent in statistics between purulent meningitis group and viral meningits group(P〈0.01). Conclusion The concentration of LDH and β2- m in CSF can be treated as rapid diagnostic indices in differentiating purulent meningitis from viral meningitis in pediartic patients. It also provide directly laboratory evidence for clinical pediatrician to diagnose the two difierent diseases.
