摘  要:目的比较6-氨基己酸(epsilon-aminocaproicacid,EACA)与抑肽酶(aprotinin)在体外循环(cardiopulmonarybypass,CPB)中抗炎症反应方面的作用,进一步探讨EACA在CPB中的应用。方法30例先天性心脏病心内直视手术患儿,分为抑肽酶组和EACA组,于CPB转机前、CPB30min和停机即刻测定血浆白细胞介素(interleukin,IL)-6及IL-10浓度。结果EACA和抑肽酶组患儿CPB开始前血浆IL-6和IL-10浓度差异无统计学意义(P>0.05)。相同时点两组比较,EACA组血浆IL-6和IL-10浓度CPB转机30min和停机即刻和抑肽酶组相同时间比较差异有统计学意义(P<0.05)。结论通过本试验结果可以认为EACA和抑肽酶在CPB中具有一定的抗炎作用,但EACA的作用较抑肽酶作用弱,可是因其价格便宜、无过敏反应等优点,值得今后临床更多地使用观察。Objective To observe and compare the influence of epsilon-aminocaproic acid with aprotinin on inflammation in cardiopulmonary bypass.To evaluate the value of epsilon-aminocaproic acid in cardiopulmonary bypass. Methods Thirty children with acyanotic congenital heart disease were enrolled in the study.The patients were randomized into two groups:aprotinin group and epsilon-aminocaproic acid group.Give different medicine to these two groups.Measured parameters include plasma interleukin-6(IL-6)and IL-10 respectively. Results In the two groups, before the CPB onset,plasma IL-6 and IL-10 respectivly,there was no difierence between them(P〉0.05).Plasma IL-6, at the same time points of 30 minutes after CPB and CPB over,there was significant difference between the two groups (P〈0.05).Plasma IL-10,at the same points there was significant difference between the epsilon-aminocaproic acid group and the aprotinin group (P〈0.05). Coliclusion In this study epsilon-aminocaproic acid is not stronger than aprotinin in anti inflammation effect for CPB patients.
