射频热凝固治疗不同程度子宫颈糜烂及相关问题的探讨

摘　　要：目的探讨射频热凝固微创技术(RHMIT)治疗不同程度子宫颈糜烂患者的疗效和相关问题。方法对4216例(Ⅰ度467例、Ⅱ度2781例和Ⅲ度968例)经宫颈刮片细胞学检查和或电子阴道镜下活检病理证实的不同程度宫颈糜烂患者,进行了RHMIT治疗和随访。结果本组在应用RHMIT和辅助治疗后,各类型Ⅰ度糜烂治疗后4周痊愈率为94·2%、6周为100%,各类型Ⅱ度糜烂治疗后4周痊愈率为74·6%、6周为100%,各类型Ⅲ度糜烂治疗后4周痊愈率为60·0%、6周为97·1%。有1243例患者随访1年以上,平均26个月。共进行了1071次宫颈细胞学检查,142次电子阴道镜下活检病理学检查和18次宫腔镜下活检,均未见不典型细胞表现。结论RHMIT是一种简单、快速和安全的方法,对不同程度的宫颈糜烂有明确疗效。应制定适合个体化治疗方案以及重视随访。Objective To investigate the curative effect and correlated issues of radiofrequency heat- coagulation minimally invasive treatment （RHMIT） on cervical erosion. Methods A total of 4216 patients with cervical erosion in different degree （467 with stage Ⅰ, 2781 stage Ⅱand 968 stage Ⅲ ）, who were all confirmed by Thin Prep cytology test or electric colposcopy with cervical biopsy pathology, were treated by RHMIT and strictly followed up. Results The curative rates of RHMIT and supplementary treatment for the patients with cervical erosion in I degree of every types were 94.2% within 4 weeks and 100% within 6 weeks, in Ⅱ degree were 74.6% within 4 weeks and 100% within 6 weeks, in m degree were recovered 60.0% within 4 weeks and 97.1% within 6 weeks. After the treatment, 1 243 patients were followed up for more than one year （26 months in average） with 1 071 times of TCT and 142 times of cervical biopsy pathology under electric colposcopy and 18 times of hysleroscopy and no atypical cells were found. conclusions RHMIT is an easymanipulated, safe, rapid and effective treatment for cervical erosion in different degree. The individual plan and strict followup program should be performed.
