人类IgA肾病肾小球Ki-67阳性细胞、α-SMA表达的免疫组化研究

摘　　要：目的探讨IgA肾病(IgAN)肾组织K i-67阳性细胞和α-平滑肌肌动蛋白(α-SMA)表达的临床意义。方法对59位IgAN患者肾组织进行K i-67、-αSMA免疫组化染色,利用显微镜及电子图像分析系统计数每个肾小球的K i-67阳性细胞数和α-SMA值,结合临床资料、24 h蛋白尿、血肌酐(Scr))和肾小球损害程度,用SPSS软件系统进行统计学分析。结果肾小球K i-67阳性细胞的表达和24 h尿蛋白(r=0.6398,P<0.01)、Scr(r=0.4543,P<0.05)及肾小球损害程度呈正相关(rs=0.4891,P<0.05);K i-67阳性细胞的表达和α-SMA的表达也呈正相关(r=0.519,P<0.001),但-αSMA的表达和24 h尿蛋白、Scr及肾小球损害程度均无相关关系。结论K i-67阳性细胞在肾小球的表达程度可作为IgAN病情进展的指标;而α-SMA在肾小球的表达仅是系膜细胞激活和增殖的敏感指标,而不是IgAN病情严重程度的可靠指标。Objective To explore the clinical significance of Ki-67-positive cells and α-smooth muscle actin （α-SMA） in human IgA glomerulonephritis. Methods Ki-67-positive cells and α-SMA immunohistochemical staining was conducted in kidney tissues from 59 patients with IgA glomerulonephritis, and cell count of Ki-67-positive cells and α-SMA values were calculated with microscopy and electronic image analyzing system, and in combination of clinical indices （24h proteinuria concentration and serum creatinine （SCr） and glomendar injury, statistic analysis was conducted with SPSS software. Results Expression of Ki-67-positive cells and 24h proteinuria concentration were positively correlated （ r = 0. 6398,P 〈 0.01 ） ; expression of SCr （ r = 0. 4543,P 〈 0.05） and glomerular injury （r =0.4891 ,P〈 0.05 ）werc also positively correlated; and expression of Ki-67-positive cells and the expression of α-SMA were also positively corrclated （r = 0. 519, P 〈 0. 001 ）. The expression of α-SMA were not correlated with 24h proteinuria concentration, SCr,and glomerular injury. Conclusion the expression of Ki-67-positive cells can be considered as the sign for the progression of IgA glomerulonephritis ; and the expression of α-SMA in glomerulus can only be considered as the sensitive signs of mesangial cells activation and proliferation rather than the reliable signs for the aggravation of lgA glomerulonephritis.
