摘 要:目的探讨常温下第一肝门阻断对肝右叶手术出血量及肝功能的影响。方法呃生总结23例肝右叶部分切除术的临床资料,分为第一肝门阻断组8例与非阻断组15例。对比2组间手术时间、出血量及手术前后肝功能的变化。结果两组间手术时间(133.8±20.6;147.5±29.4min)和出血量(357.1±207.8;275±238.2mL)无明显差别。未阻断组手术前后AST(39.8±8.5;79±23.1,P<0.01),ALT(45.38±35;136±56,P<0.05)明显升高,而阻断组变化不显著。结论常温下第一肝门阻断对肝右叶手术的手术时间和出血量无影响,但短时间肝门阻断可能对肝功能有保护作用。Objective To evaluate the effect of hepatic hilar ocelusiono on blood loss and liver function in partial who under went right lobe liver resection. Methods The date from 23 patients with partial right liver resection were reviewed. Eight were included in occlusion group and 15 were in non occlusion group. Operation time, the amount of blood loss, the change of aspartate aminotransferase(ASL),and aniline aminotransferase(ALT)were compared between the 2 groups. Result No change was found in operation time (133.8±20.6;147.52±29.4min)and the amount of blood loss(357.1±207.8;275±238.2 mL)Between two groups. However, the AST and ALT significantly increased in the non-occlusion group compared with the occlusion group. Conclusion The hepatic hilar occlusion has no effect on the operation time and the blood loos in partial right lobe liver resection,but could prevent the elevation of AST and ALT level.
