摘  要:目的探讨血清炎性因子白介素-6(IL-6)、白介素-8(IL-8)、白介素-10(IL-10)及肿瘤坏死因子(TNF)在多发伤中所起的作用.方法多发伤、单发伤各20例,20例健康者作为正常对照组,分别在住院第1、3、5、7、9天用放射免疫方法检测IL-6、IL-8、IL-10及TNF.结果多发伤患者血清中IL-6、IL-8、及TNF均明显升高,在伤后3~5 d为高峰,5 d后开始降低,多发伤组与其他两组相比差异非常显著(P<0.01),而单发伤组峰值较低,为一过性.多发伤患者血清中IL-10在伤后72 h达高峰,第6~7天逐渐降低,各组相比差异非常显著(P<0.01).结论IL-6、IL-8、TNF属促炎性细胞因子,IL-10属抗炎性细胞因子,在多发伤的病情演变中都起着重要的作用,创伤发生后3~5 d是治疗的关键,及时监测IL-6、IL-8、IL-10、TNF的变化,同时监测生命体征、多项生化指标,可更敏感地观察多发伤患者的治疗效果,预测病情发展.Objective To investigate the effect of inflanunatory cytokines interleukin-6 (IL-6), interleukin-8 (IL-8),interleukin-10 (IL-10) and tumor-necrosis factor (TNF) in multiple injury. Methods Multiple injury (20 cases) ,single injury (20 cases),controls (20 healthy cases) were detected for IL-6,IL-8,IL-10,TNF in serum at 1, 3,5,7 and 9 day respectively by radioimmunoassay.Results IL-6,IL-8 and TNF in serum of multiple injury patient increased significantly, the crest was 3-5 d later after trauma,and decreasing 5 days later,the difference among the groups was significant ( P 〈 0.01),the peak value in single injury group was low and transient. IL-10 in serum of multiple trauma patient attained the high level at 72 h after the trauma, and decreased gradually at 6 - 7 d, the difference was significant ( P 〈 0.01). Conclusion IL-6, IL-8 and TNF are pro-inflammatory cytokines, IL-IO is anti-in- flammatory cytokine,they play an important role in the development of diseases,it is the key time of treatment at 1 - 5 d after trauma. Monitoring IL-6, IL-8, IL-10, TNF in serum in time, and monitoring life sign, biochemical parameters at the same time could sensitively observe the effect of treatment and forecast the trend of diseases in multiple injury patients.
