充血性心力衰竭心气虚、心阳虚证与TNF-α、IL-6及IL-10的关系

摘　　要：目的探讨充血性心力衰竭(CHF)患者心气虚、心阳虚证与肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及白细胞介素-10(IL-10)的关系。方法选择CHF心气虚、心阳虚证住院患者62例,分别进行NYHA心功能分级,并行心脏多普勒、血清TNF-α、IL-6和IL-10等检查。结果心气虚证组患者心功能分级以舒张功能不全和心功能Ⅰ级、Ⅱ级为多,心阳虚证组患者以心功能Ⅲ级、Ⅳ级为多;心气虚证患者E/A值、EF值高于心阳虚证;心阳虚证患者血清TNF-α、IL-6水平高于心气虚证;心阳虚证患者血清IL-10水平低于心气虚证。结论TNF-α、IL-6与IL-10可能在CHF心气虚证向心阳虚证发展过程中起重要的作用,可以作为心气虚证、心阳虚证微观辨证的指标。Objective:To investigate the relationship between the content of TNF- α , IL- 6, IL- 10 and the Syndrom of Insufficiency of Heart Qi,of Insufficiency of Heart Yang in Patients with CHF.Methods:62 patients with CHF were selected and divided into the Syndrom of Insufficiency of Heart Qi group and the Insufficiency of Heart Yang group.Then the grade of heart failure was assessed according to NYHA.The heart of all patients was detected by dopler and the content of TNF- α ,IL- 6 and IL- 10 in their sera were measured.Results:The heart failure grade of patients in the Syndrom of Insuffeciency of Heart Qi group was Grade Ⅰ or Grade Ⅱ ,while that in the Insufficiency of Heart Yang Deficiency Syndrome group was Grade Ⅲ or Grade Ⅳ with very significant difference with the former.The value of E/A and EF of the Heart Qi Deficiency Syndrome group was higher than that of the Heart Yang Deficiency Syndrome group,and the result was quite opposite about the content of TNF- α and IL- 6,and same as the content of IL- 10.Conclusion:TNF- α , IL- 6 and IL- 10 play a very important role during the development from the Syndrom of Insufficiency of Heart Qi to of Insufficiecy of Herat Yang Deficiency,and perhaps they can be used as indexes for the micro- bianzheng.
