小腿及足踝部软组织缺损的显微外科治疗

摘　　要：目的：探讨小腿及足踝部的软组织缺损的治疗效果。方法：应用吻合血管的游离皮瓣、肌皮瓣移植修复。结果：本组共５６例，皮瓣、肌皮瓣种类达７种，经随诊半～６年。除２例术后发生静脉栓塞皮瓣坏死，１例肿瘤复发截肢外，其余５３例皮瓣质地良好，功能改善。结论：应用游离皮瓣或肌皮瓣移植修复，修复面积大，血供丰富，促进愈合过程，对合并骨髓炎，慢性质疡的缺损修复更是较好的适应证，按不同部位缺损修复要求，应选择不同的皮瓣或肌皮瓣修复。Objective:to present the treatment of the soft-tissue defects of the leg and heel.Methds:used seven kinds of free flaps and musculocutaneous flaps by vessels anastomosis in 56 cases。Results:follow-up 6 moths to 6 year. the result are satisfactory and functional improvement in 53 cases. two free flaps are necrosis and one case had to be amputated in leg due to recurrence of the cancer,Condusious:this technique should be first used to repair the soft tissue defect of the leg and heel especially complicated osteomylites and chronic ulcers,because the flap has excellent blood supply and can be provided larger donor for repair.
