MED治疗腰椎间盘突出症中出血发生原因的初步探讨

摘　　要：初步探讨后路显微椎间盘镜(MED)治疗腰椎间盘突出症手术中出血的原因。回顾性分析104例行MED治疗腰椎间盘突出症的术中出血发生情况。结果表明,发生术中出血4例,发生率为3.8%。4例均为椎管内血管破裂出血,全组无术后因出血过多二次手术、感染及死亡。由此可知,患者术前接受治疗程度、体重、腹围等因素及手术者熟练程度、经验和适应症的选择与MED术中出血有关。遵守操作原则,熟悉椎管内解剖,充分的术前准备可减少术中出血的发生率。Object：to investigate the causes of vessel bleeding in microendoscopic discectomy system（MED）. Methods 104 cases were treated with MED. The retrospective analysis was used among the problem of bleeding in these cases. Results 4 cases （3.8%） showed lumbar vertebral canal＇s blood vessel bleeding. There were no second operation, postoperative infection and death. Conclusion Vessel bleeding in MED are related to various factors. The preoperational treatments, patients＇ weight, surgeons＇ experience of MED manipulation, the right indication, the strict observance of rules and skillful with the anatomic structures are important in decreasing the vessel bleeding.
