摘  要:观察59例维持性血液透析(MHD)、36例持续性非卧床性腹膜透析(CAPD)病者透析前后血脂、脂蛋白及载脂蛋白的变化。结果显示,MHD病者透析后血甘油三脂(TG)、极低密度脂蛋白胆固醇(VLDL-C)轻度升高,载脂蛋白B-100(apoB-100)明显升高,总胆固醇(TC)、apoAI、apoAⅡ、apoCⅡ及apoCⅡ/apoCⅢ均明显降低;CAPD病者治疗后TG、低密度脂蛋白胆固醇(LDL-C)、VLDL-C、apoB-100、apoCⅢ较透析治疗前明显升高,而apoAⅠ明显降低;有动脉硬化合并症的透析患者,其脂质代谢紊乱明显重于无动脉硬化的患者。上述事实表明,透析病者有明显的脂质代谢紊乱,CHD和CAPD对透析病者脂质代谢的影响有不同,且两者均存在加速和对抗动脉硬化因子的失衡,有明显的促进动脉硬化的倾向。In order to understand the lipid metabolism in patients receiving dialysis , the plasma lipid , lipoproteins and apolipoproteins of 59 patients with maintain hemodialysis and 36 with CAPD were measured. The results showed that TG , VLDL-C and were lightly increased , apo B100 was markedly increased in the patients undergoing hemodialysis , whereas TC , apo AⅠ , apoAⅡ , apoCⅡ , apoCⅡ/apoCⅢwere decreased. In patients withCAPD, raised TG , LDL-C , LDL-C apo B-100 , apoCⅢ and reduced apo AⅠ were observed. Meanwhile , the authers also found the different level of lipids, liproteins , apolipoproteins in the patients with and without arterosclerosis. All data suggested that there were markedly lipid metabolism deturbance in patients receving hemodialysis and CAPD. The imbalance between anti- and accelerated arterosclerosis factors may be the main cause inducing atherosclerosis.
