摘  要:目的探讨肺表面活性物质(PS)对支气管肺发育不良(BPD)的预防和治疗意义。方法将20例早产儿呼吸窘迫综合征(RDS)机械通气的患儿按是否早期使用PS分两组,观察病情的转归及BPD的发生情况,并对8例BPD患者按是否使用PS治疗分两组,观察两组呼吸机依赖例数及转归,分析和对比使用PS前后的临床表现、动脉血气。结果早期使用PS组机械通气的时间及BPD的发生率均较对照组有显著差异(P<0.05),且BPD患者使用PS后的动脉血气、氧和指数较使用前明显改善,差异显著(P<0.05,P<0.01)。结论PS可以改善肺的顺应性及氧和功能,对BPD的预防和治疗具有积极的意义。Objective The aim of this study was to determine the therapeutic effect of pulmonary surfactant (PS) on the development of bronchopulmonary dysplasia (BPD) in premature infants. Method Twenty premature infants with respiratory distress syndrome (RDS) were divided into 2 groups. Eight subjects with RDS were selected into treatment and control groups. BPD occurrence rate as defined by the ventilator dependence rate, clinical parameters and arterial blood gas level in each group before and after treatment was observed. Result The mechanical ventilation time and the incidence of BPD in the PS group were significantly different from the control group (P〈0.05). The infants with BPD also had better arterial oxygen tension and saturation after PS treatment, and the difference was significant (P〈0.05, P〈0.01). Conclusion PS has both preventive and therapeutic effect on pre-mature infants with BPD. The pulmonary function can be improved by PS.
