摘  要:目的:探讨血尿酸(SUA)对急性心肌梗死(AM I)发生、发展及预后的影响,以及与心血管其他危险因素的相关性。方法:选择AM I 36例,不稳定型心绞痛(UAP)40例,稳定型心绞痛(SAP)40例,正常对照组(N or)40例。AM I组病人用酶法测定心肌酶,用T rinder法测定SUA。而UAP,SAP,N or组分别于入院即刻,第2 d清晨空腹抽取静脉血测得心肌酶,SUA值。对所有病人进行空腹血糖,血脂,外周血白细胞的测定和心电图检查。结果:在N or,SAP,UAP,AM I四组,SUA逐渐增高,四组SUA相比较,仅UAP与AM I组相比无统计学意义(P>0.05),余各二组间相比均有显著性差异(P<0.001)。在AM I组,SUA浓度随时间呈逐渐下降趋势。相关分析:SUA与体重,CRP,心功能分级(NYHA分级)呈正相关,与射血分数(EF)呈负相关;与空腹血糖,甘油三酯无相关性。结论:SUA随着CHD的发展而增高,可能参与动脉硬化的形成,影响了AM I的发生,发展和预后,且对其发展和预后有判断和预测价值。Objective: To investigate the relationship between the levels of serum uric acid (SUA) and acute myocardial infarction (AMI). To observe the relationship between the levels of SUA and the major risk factors of AMI. Methods: Serial measurements of SUA and creatine kinase (CK) level every 6 hours were performed in 36 patients. The CRP, SUA and CK concentration were tested in 40 unstable angina pectoris (UAP), 40 stable angina pectoris (SAP) and 40 normal control (Nor). To measure the values of blood glucose, blood lipid and white cell count and make a electrocardiogram (ECG) in all patients. Results: The SUA concentration were gradually increasing from Nor group, SAP group, UAP group, and AMI group. There were significant difference in each other but between the SUA level of AMI and UAP. Correlation analysis: there were positive relationship between SUA and weight, blood lipid, heart NYHA grades. There were negative relationship between SUA and ejection fraction. Conclusion: The levels of SUA were gradually increasing according to the disease degrees and could be used as a predictor of the prognosis of AMI.
