声学造影评价左心室质量的实验研究

摘　　要：目的评价心肌声学造影测定左室心肌质量的可行性与准确性。方法40只大鼠随机分为两组,其中一组为对照组,另一组用Anversa方法制成左室心肌肥厚模型(LVH组),备实验用。白蛋白氟碳微泡造影剂,0.2ml静脉推注。造影前用M-型超声心动图测定心肌质量。造影后以截椭圆体法,通过勾画左室内外膜测定左室质量。造影前后,超声测得的心室质量与解剖测值比较。结果在左室肥厚组,造影前后测得的左室质量分别为(1.08±0.11)g和(0.95±0.06)g,解剖测值为(0.98±0.05)g。超声造影后测得的左室质量与解剖测值的相关性良好(r=0.96)。结论心肌声学造影能可靠准确地测定左室质量。Objective To evaluate the feasibility and accuracy of myocardial contrast echocardiography （MCE） for measuring left ventricular mass （LVM） compared with anatomic specimens. Methods Fourty Wister rats were randomly and equally divided into model group and the control group. Using Anversa method （abdominal aortic constricture） to make left ventricular hypertrophy （LVH）, every rat was proved to have LVH successfully （LVH group ） after 6 weeks. The other 20 rats were examined to exclude LVH as the control group. 2D and M-mode echocardiography was performed in the two groups before and after intravenous infusion of a perfluorocarbon contrast agent. Before contrast, LVM was measured by M-mode echocardiography. The epicardium and endocardium were delineated using MCE. endocardial and epicardial volumes were calculated via the truncated ellipsoid （TE） method. Results In the model （LVH） group, anatomic measurement of total LVM was 0.98±0.05g, whereas echocardiographic calculation of LVM before and after intravenous contrast agent was 1.08±0. 11g and 0.95 ± 0. 06g respectively. A significant correlation was observed between MCE estimates of LVM and the corresponding anatomic measurements （r=0.96） . LVM in LVH group and the control group was 0.98±0. 05g and 0.73±0.04g respectively （P 〈 0.05 ）. Conclusions LVM derived from MCE showed a significant correlation with the anatomic results. Visualization of the endocardial border was better on MCE, which would provide a more accurate and reliable means for determining LVM in the experiment.
