摘 要:目的研究正常成人颅内动脉的显示率,评价Willis环前、后交通动脉循环功能。方法256例正常成人按年龄男女均各分3组,采用经颅多普勒血流显像(TCCDFI)、能量多普勒(CDE)及超声造影(ECE)技术,观察颅内动脉走行及血流方向。并通过压迫颈总动脉(CCA),观察大脑前动脉(ACA)、后动脉(PCA)血流方向及血流速度的变化,判断Willis环前、后交通动脉循环功能。结果(1)颅内动脉成功显示率男女均随年龄增长而下降。3组女性间有明显差异,老年女性明显低于老年男性(P<0.05)。Willis环完全未显示的老年组被检者行ECE仍有85%可清晰显示;(2)前交通动脉功能开放率男女分别为87.6%、83.3%,后交通动脉左侧分别为28.9%、34.1%,右侧分别为34.4%、29.2%。结论TCCDFI、CDE及ECE能实时观察颅内动脉走行及血流方向,评价前、后交通动脉循环功能,可作为颅内动脉检查的有效方法。Objective To study the identification rates of normal adult cerebral arteries, and to evaluate the functional patency of the anterior and posterior communicating arteries (AcoA and PcoA respectively) of the circle of Willis. Methods Totally 256 normal adults were divided into three groups according to aging. The course and blood flow direction of the cerebral arteries were observed via transcranial color Doppler flow imaging (TCCDFI), color Doppler energy (CDE) and echo-contrast enhanced agents (ECE) ; and the functional patency of the AcoA and PcoA was assessed by the changes of blood flow velocity and direction (ACA, PCA) during common carotid artery (CCA) compression. Results 1. The identification rates of cerebral arteries decreased both in men and women with the increase of age. There were significant differences among women in the three groups, and between men and women in the third group (P〈0.05) . The unvisited arteries of the circle of Willis with TCCDFI in the third group could be clearly revealed after ECE. 2. The rates of the functional patency of the AcoA for men and women were 87.6% and 83. 3%, left PcoA 28. 9% and 34.1%, right PcoA 34.4% and 29.2%, respectively. Conclusions The course and blood flow direction of their cerebral arteries can be observed real-timely. The functional patency of AcoA and PcoA can be evaluated by TCCDFI, CDE and ECE. Integration of the three method should be an effective means for cerebral artery examination.
