摘  要:目的探讨螺内酯对单侧输尿管梗阻(UUO)大鼠肾间质纤维化的影响。方法2004-12~2005-03对UUO大鼠给予螺内酯20mg·kg-1·d-1及50mg·kg-1·d-1灌胃,观察第14天血清钾、钠、肌酐、血压的变化及肾间质纤维化程度、转化生长因子-β1(TGF-β1)、α-平滑肌肌动蛋白(α-SMA),纤维连接蛋白(FN)的表达。结果与UUO组相比,两治疗组血清钾、钠、肌酐、血压差异均无显著性(P>0.05),肾小管间质纤维化程度,TGF-β1、α-SMA、FN的表达与UUO组差异有显著性(P<0.05),两治疗组之间差异无显著性(P>0.05)。结论螺内酯可通过抑制细胞因子释放和肾小管-间质细胞的表型转化等非血压依赖机制延缓肾间质纤维化。Objective To study the preventive effects of spironolactone on renal tubulointerstitial fibrosis induced by unilateral obstruction (UUO) in rats. Methods The experiment was successfully conducted from December, 2004 to March, 2005 in Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology. Rats subjected to UUO were administered orally a daily dose of spironolactone (20 mg/kg or 50 mg/kg) by gastric gavage. Serum potassium, sodium, creatinine and blood pressure were measured on the 14th day after operation. The degree of tubulointerstitial fibrosis and the expression of TGF - β1, α- smooth muscle actin ( α - SMA ), fibronectin (FN) were evaluated and compared. Results In comparison with UUO group, serum potassium, sodium, creatinine and blood pressure were analogous ( P 〉 0. 05 ), but the degree of renal tubulointerstitial fibrosis, interstitial expression of TGF -β1 , α - smooth muscle actin (α - SMA) and FN were significantly reduced in both low and high dose of spironolactone - treated group (P 〈 0. 05 ) . There was no significant difference in the degree of renal tubulointerstitial fibrosis, interstitial expression of TGF - β1,α - SMA and FN between low and high dose of spironolactone - treated group ( P 〉 0.05 ) . Conclusion Spironolactone ameliorates renal fibrosis presumably via the inhibition of cytokine release and tubular - epithelial - myofibroblast transdifferentiation independent of blood pressure.
