全自动血细胞分析仪对形态异常血细胞检测功能的评价

摘　　要：【目的】探讨雅培Cell-Dyn 3700型全自动血细胞分析仪(简称CD3700)对形态异常细胞检测功能的临床应用价值。【方法】利用该仪器和显微镜计数法(简称镜检)对490例患者标本进行分析测定和细胞分类。【结果】以镜检结果为标准,CD3700分析仪对形态异常细胞检测功能的特异性为77.7%,敏感性为100%,检出有效率为83.7%,假阳性22.3%,假阴性0%,阳性预示值62.3%,阴性预示值100%。未成熟粒细胞/幼稚细胞、有核红细胞、异形淋巴细胞的符合率分别为49.5%、33.3%、58.6%。【结论】CD3700分析仪对血细胞的检测具有良好的技术性能,对形态异常细胞提示的敏感性高,重复性较好,但还不能完全替代显微镜观察。[ Objective] To clinically evaluate the Abbott Cell-Dyn 3700 automatic blood cell analyzer in detecting abnormal morphologic cells. [ Methods] Four hundred and ninety specimens were counted by Abbott Cell-Dyn 3700 Analyzer and classified by direct microscope enumerative method. [ Results] With the results of microscope enumerative method as the reference, the specificity of the analyzer for abnormal morphologic cells was 77.7%, the sensitivity was 100%, the detectable rate was 83.7%, the false positive rate was 22.3%, the false negative rate was 0%, the positive predictive value was 62.3% and the negative predictive value was 100%. The coincidence rates of immature granuloeytes/blasts, nucleated eiythroeytes, heteromorphic lymphoeytes were 49.5%, 33.3% and 58.6% respectively. [ Conclusion] The technical performance of Abbott Cell-Dyn 3700 automatic blood cell analyzer is good for the detection of abnormal morphologie cells. This analyzer has a higher sensitivity and better repeatability. However, it is no substitute for the microscope.
