摘 要:目的了解新生儿脐部感染的病原菌种类、分布及对抗生素的耐药状况,为临床用药提供指导。方法对2001年1月-2005年10月我院新生儿科140名脐炎患儿的脐部分泌物进行细菌培养、鉴定及抗生素敏感试验。试验方法严格按照NCCLS新标准进行。结果新生儿脐部感染以革兰氏阳性菌为主,占56.73%。主要致病菌为金黄色葡萄球菌、表皮葡萄球菌和大肠埃希菌。医院感染以凝固酶阴性的表皮葡萄球菌为主,占30.77%;社会感染以凝固酶阳性的金黄色葡萄球菌为主,占36.49%。主要致病菌对常用抗生素多重耐药。大肠埃希菌和肠杆菌依次对第三代头孢菌素、喹诺酮类、氨基糖甙类和磷霉素耐药率降低;葡萄球菌对万古霉素敏感,但已发现1株耐万古霉素的表皮葡萄球菌;大肠埃希菌和肠杆菌对亚胺培南敏感。结论重视和加强新生儿科医院感染的监控,严格按照微生物检测结果合理使用抗生素有非常重要的意义。Objective To understand and identify the category of pathogen of neonate navel infection and its distribution and drug resistance to antibiotics, and to provide guidances on clinic drug administration. Methods Bacteria culture, appraisal and antibiotic sensitivity trials for infected navel excretion of 140 newbom cases were conducted from January 2001 to October 2005, and the experiments strictly followed the update standards of NCCLS. Results The group of neonate navel infection is mainly due to gram - poskive bacteria, which accounts for 56,7 %. The principal pathogens are S. athens, Staphylococcus epidermitis, and Escherichia coli. Hospital infection is characterized by coagulase - negative staphylococci epidermitis, which takes up 30.77 %, and social acquired infection is largely caused by coagulase - positive S. aurens, it makes for 36.49 % .The principal pathogens show multiple resistances to conventional antibiotics. The drug resistance of Escherichia coil and Enterobacter to Cefpodoxime proxetil, qunolones and aminoglycosides and phosphomycin decreases in turn. Staphylococcus is sensitive to Vancomycin However, 1 strain of Staphylococcus epidemidis resistant to vancomycin is discovered. Escherich/a coli and enterobacter are sensitive to imipenem. Conclusion It is of significance in attaching much importance to and strengthening monitoring and control of hospital infection at Neonate departments and in guiding rational clinic antibiotics administration strictly based upon the microorganism testing results.
