摘 要:目的探讨乌司他丁对体外循环(CPB)期间肺组织ICAM-1表达的影响。方法随机将健康成年犬分为对照组(C组,n=6)和乌司他丁组(U组,n=6)。建立CPB模型,两组均在转机前(T1)、阻断升主动脉15min(T2)、45min(T3),开放30min(T4)、60min(T5)取肺组织和静脉血。测血浆黄嘌呤氧化酶(XOD)含量、肺组织髓过氧化物酶(MPO)的活性,肺组织细胞间粘附分子-1(ICAM-1)的免疫组化染色并进行图象分析及光镜检查。结果CPB后,U组各时点血浆XOD含量、肺组织MPO活性及肺组织ICAM-1的平均灰度值明显低于C组(P<0.01)。结论乌司他丁可能通过抑制肺组织ICAM-1的表达,减少中性粒细胞在肺组织内聚集及氧自由基的生成,从而减轻CPB后的肺损伤。[Objective] To study the effect of ulinastatin on the expression of ICAM-1 of lung tissue during cardiopulmonary bypass in dog and its protective effect on lung tissue. [Methods] 12 adult hybrid healthy dogs were randomly divided into control group (group C, n=6) and ulinastatin group (group U, n=6). The model of CPB in dog was established. The levels of myeloperoxidase (MPO) in lung tissue and xanthine oxidase (XOD) in plasma were detected at the time before CPB (T1), 15 min (T2) and 45 min (T3) after cross-clamping of aorta, and 30 min (T4), 60 min (T5) after aortic unclamping. Specimens of lung tissue were taken for optical microscope examination and the ICAM-1 of lung tissue was examined with immunohistochemical technique and image analysis. [Result] After CPB, the levels of MPO in lung tissue and XOD in plasma and the average grey of ICAM-1 were lower significantly in group U than that in group C at all the stages (P 〈0.01). [Conclusion] Uhnastatin can reduce the aggregation of neutrophil and the production of free radical in the lung tissues through its effect to inhibit the expression of ICAM- 1 in the lung tissues. Uhnastatin may exert a protective effect on lung injury during cardiopulmonary bypass.
