摘 要:目的探讨病毒性脑炎(病脑)患儿脑脊液(CSF)和血清S-100β蛋白变化的临床意义。方法采用酶联免疫吸附试验双抗体夹心法对36例病脑和20例无神经系统疾病而需外科手术的腰麻患儿CSF和血清S-100β蛋白进行测定。比较昏迷组与无昏迷组、惊厥组与无惊厥组、有后遗症组与无后遗症组CSF和血清S-100β蛋白水平的差异。结果 1.病例组和对照组CSF 中S-100β蛋白水平分别为(0.641±0.390)、(0.037±0.014)μg/L,血清S-100β蛋白水平分别为(0.444±0.257)、(0.023± 0.009)μg/L,两组CSF、和血清S-100β蛋白水平均有显著性差异(P<0.01,0.05);昏迷组CSF和血清S-100β蛋白均显著高于无昏迷组(P均<0 01);惊厥组CSF和血清S-100β蛋白水平均高于无惊厥组(P均<0.01);后遗症组CSF和血清S-100β蛋白均高于无后遗症组(P均<0.01)。结论病脑患儿CSF和血清S-100β蛋白水平与疾病严重程度相关,检测病脑患儿S-100β蛋白水平的变化,有助于判定脑组织受损的严重程度及评估患儿的预后。Objective To explore the changes of S - 100β protein in cerebrospinal fluid and serum of children with viral encephalitis and its clinical significance. Methods The levels of S - 100β protein of cerebrospinal fluid and serum of 36 children with viral encephalitis and 20 lumbar anesthesia children without central nervous system diseases were measured by enzyme - linked immunosor bent assay. Differences in the levels of cerebrospinal fluid and serum S - 100β protein between children with and without coma, with and without convulsion, with and without sequelae in the ease group were compared. Results S - 100β protein levels of eerebroepinal fluid in the ease group and control group were (0.641±0.390) and (0.037±0.014)μg/L(P 〈 0.01). Serum S - 100β protein levels in the case group and control group were (0.444±0.257) and (0.023±0.009)μg/L(P 〈 0.05).The cerebrospinal fluid and serum S - 100β protein levels of children with coma were significantly higher than those of children without coma (all P 〈 0.01), and those of children with convulsion were higher than those of children without convulsion (all P 〈 0.01). The cerebrospinal fluid and serum S - 100β protein levels of children with sequelae were significantly higher than those of children without sequelae (all P 〈 0.01). Conclusions The cerebrospinal fluid and serum S - 100β protein levels of children with viral encephalitis relates to degree of severity. Detection of S - 100β protein of children with viral encephalitis contributes not only to assess the severe degree but also to predict their prognoses.
