应用不同内固定方法治疗髌骨折68例分析

摘　　要：目的探讨不同方法治疗髌骨折的最佳适应证。方法采用环扎固定法、改良AO张力带钢丝、记忆合金聚髌器治疗髌骨折共68例。结果观察66例,其中最短随访5个月,最长4年;环扎固定法10例,优4例,良3例,可3例;改良AO张力带钢丝固定42例,优36例,良4例,可2例,记忆合金聚髌器固定14例,优12例,良1例,可1例。结论改良AO张力带法与记忆合金聚髌器较环扎固定法疗效好,改良AO张力带法与记忆合金聚髌器之间疗效差异无统计学意义(P>0.05),环扎固定法治疗对膝关节功能有影响。Objective To explore the optimum indications of different fixative methods in the treatment of patellar fracture. Methods 68 eases with patellar fracture were treated with loop - lock fixation, improved AO tension-band method and Nickel-Titanium patellar concentrator （NT- PC）, respectively. Results 66 cases were followed up from 5 months to 4 years. Of 10 cases treated with loop - lock fixation, 4 cases were excellent, good in 3 cases and fair 3 cases. Of 42 cases treated with AO tension-band fixation, 36 cases were excellent, good in 4 cases and fair 2 cases. Of 14 cases treated with NT- PC fixation, 12 cases were excellent, good in 1 case and fair 1 case. Conclusion The curative effect of improved AO tension - band fixation and NT - PC fixation is better than that of loop - lock fixation; the difference of curative effect is not significant between improved AO tension- band method and NT- PC method.
