血浆脂联素、瘦素、C反应蛋白与胰岛素抵抗的关系

摘  要:目的观察2型糖尿病(T2DM)患者、糖调节受损(IGR)患者和糖耐量正常(NGT)者血清脂联素、瘦素水平等指标与胰岛素抵抗指数(HOMA2 IR)的关系,探索各种因素对胰岛素抵抗的影响。方法使用酶联免疫吸附试验测定血清中胰岛素、脂联素、瘦素、C反应蛋白等指标水平,计算胰岛素抵抗指数,利用多元回归等统计方法观察各项指标对胰岛素抵抗指数的影响。结果T2DM组的血清脂联素水平明显低于IGR和NGT组;血清瘦素和C反应蛋白水平明显高于IGR组和NGT组。胰岛素抵抗指数与脂联素水平呈负相关,与瘦素、C反应蛋白水平呈正相关。结论脂联素、瘦素、C反应蛋白是影响HOMA2 IR的重要因素,其中脂联素可以显著改善胰岛素抵抗。Objective To investigate the relationship of the adiponectin, leptin and C reactive protein (CRP) with the index of insulin resistance in diabetes group , impaired glucose regulatory group (IGR)and normal glucose tolerance group (NGT). Methods Analyzed the levels of adiponectin , leptin in plasma with the insulin resistance index by stepwise regression analysis. Results Adiponeclin level in plasma in diabetes group was lower than that in IGR and NGT group. Leptin and CRP level was higher than that in IGR and NGT group. The index of insulin resistance was negatively correlated with the level of adiponectin and positively correlated with the level of leptin and CRP in plasma. Conclusion Adiponectin, leptin and CRP are important factors that influencing insulin resistance. Adionnectin could imnrove the insulin resistance significantly.
