摘 要:目的测定充血性心力衰竭(CHF)患者QT离散度(QTd)的变化,探讨倍他乐克对CHF患者QTd和预后的影响.方法230例心功能Ⅱ~Ⅲ级的CHF患者随机分为观察组和对照组,在常规治疗的基础上,观察组给予倍他乐克治疗,对照组仅给予常规治疗,入选和随访结束时同步描计12导联心电图,测定QTd.结果入选时,观察组和对照组患者的脉搏(AP)、左室射血分数(LVEF)、QTd的均值间差别均无显著性差异(P>0.05).随访结束时观察组AP、QTd明显降低(P<0.001).对照组治疗后AP、QTd均增加(P<0.001).治疗后两组患者的AP、QTd的均值间差别有显著性意义(P<0.001),两组患者的死亡率和心功能恶化发生率间差别均无显著性意义(P>0.05).结论倍他乐克可明显降低CHF患者的QTd,改善CHF患者心功能和预后.Objective To investigate the long term effects of betaloc on QT dispersion (QTd) in patients with congestive heart failure (CHF). Methods 230 NYHA class Ⅱ-Ⅲ CHF patients were randomly divided into the observation group and the control group, Both groups were treated with routine drugs, while the observation group added betaloc. QTd were measured before and after the treatment of betaloc. Results No significant difference were found among the parameters including arterial pulse (AP), left ventrieular ejection fraction (LVEF), QTd ( P 〉0.05 ) between beth the groups at the moment of enrollment. At the end of following up, AP and QTd were markedly decreased in the observation group ( P 〈0.001). But AP and QTd were increased significantly ( P 〈0. 001 ) in the control group. Great difference ( AP, QTd) were found between the two groups after treatment. There is no significant difference in the mortality rate between the observation group and the control group ( P 〉 0. 05 ). Conclusion Betaloc treatment is associated with a decreased QTd. It suggests that both the heart function and the prognosis may be improved in CHF patients by the beta - blocker treatment.
