肌钙蛋白Ⅰ和高敏C反应蛋白与急性冠状动脉综合征的临床研究

摘　　要：目的通过肌钙蛋白Ⅰ和高敏C反应蛋白水平探讨两种标志物与急性冠状动脉综合征的关系,及其在急性心肌梗死(AMI)中发生心脏事件的特点。方法受试者共113例,SAP 40例、UAP 43例、AMI 30例,对照组35例。cTnI采用荧光标记免疫测定;hs-CRP采用散射比浊法检测。结果①UA组cTnI明显高于SAP组及对照组(P<0.01);②AMI组明显高于UAP组(P<0.01);(1)AMI组hs-CRP与其它组相比显著增高,差异有显著意义(P<0.05),(2)UAP组与SAP组相比差异有统计学意义(P<0.01),与对照组相比差异有统计学意义(P<0.01),(3)而SAP组与对照组相比差异无统计学意义(P>0.05)。结论AMI时cTnI增高显著,是诊断急性心肌损伤的“金标准”;AMI时hs-CRP增高显著,表明AMI过程中存在炎症反应,并在一定程度上反映心肌损伤及坏死的严重程度。而联合评估这两项标志物的水平则可以识别心脏高风险的病人。Objective To explore the correlation between the cardiac troponin I level with high sensitive C-reactive protein with acute coronary syndrome, and investigate characteristics of cardiovaseulat events in acute myocardial infarction（AMI）. Methods One hundred and ten-three cases were studied,SAP group （40 patients） ,UAP group （43 patients） ,and AMI group （30 patients）. There were 35 cases in normal control group. The cTnI adoption fluorescence immtmity measurese;The hs-CRP adoption particle-enhanced immunonelometry. Results cTnI significantly increased in U AP group compared with normal group and SAP group （0. 293 ± 0.055：0.061 ± 0.011：0.042 ± 0.007, P 〈 0.01）AMI（8.75 ± 7.92） ng/L is obviously high in the UAP （0.293 ± s 0.055） ng/L（ P 〈 0.01） .The hs-CRP is obviously high in the AMi and other group （ P 〈 0.05） ,The hs-CRP in UAP group was higher compared with SAP group（ 10.53 ± 7. 875,2.22 ± 1.511 ）. The hs-CRP in SAP group was not significantly increased compared with normal control group（2.22 ± 1.511,1.51 ± 0.721 ）. Conclusion To be ＂the gold standard＂, The obvious increase of cTnI can predict the myocardial injury in AMI. Significant elevation of hs-CRP suggests thai inflammation in AMI courses and reflects certain degeee of the myocardial injury and necroses. But the unite evaluate the patient whom these two levels of marking things can indentify high risk of heart.
