摘 要:目的探讨C-反应蛋白(CRP)水平与基因型对急性心肌梗死(AMI)预后的影响。方法采用免疫比浊法检测151例AMI患者血清高敏CRP含量,聚合酶链反应-限制性片段长度多态法检测其CRP1059G/C基因型,对照组为80例健康者。观察AMI患者冠状动脉狭窄程度,预后情况并进行分析。结果AMI患者基因型分布:GG型119例,GC型25例,CC型7例。基因型分布与对照组间差别无显著性意义(P>0.05)。3型患者入院时、入院后1周及2周时CRP水平及持续时间间差别均有显著性意义(P<0.01)。不同CRP基因型的AMI患者冠状动脉狭窄程度间差别有显著意义(P<0.05)。不同CRP基因型的AMI患者预后情况间差别亦有显著性意义(P<0.01)。结论AMI患者CRP基因型分布与健康人群无差别。GG基因型者CRP水平高于GC、CC型,冠状动脉狭窄程度亦较其他两型严重,且预后差。AMI患者的预后可能受CRP基因型影响。Objective To evaluate the effect of gene 1059G/C polymorphism of plasma C -reactive protein (CRP) on the prognosis of acute myocardial infarction. Methods Plasma CRP was measured by ultrasesitive immunoturbidimetic analyzer in 151 patients with acute myocardial infarction and the CRP gene 1059G/C polymorphism were screened by PCR - RFLP. 80 healthy persons were in the control group. The degree of coronary artery stenosis and the prognosis were observed and analyzed. Results The genotype freguencies of CRP gene were consistent with that predicted by Ward - Weinbery equilibrium in the population of present study. GG genotype carriers were 119, GC carriers were 25, and CC carriers were 7, There was no significant difference among the three groups of patients in CRP gene 1059 G/C polymorphism ( P 〉0. 05 ). There was significant difference among the three genotypes ( P 〉0. 01 ). There was relationship between coronary artery, stenosis and CRP genotype. There was significant difference among the three genotypes ( P 〈0. 05 ). There was significant difference among the prognosis of the AMI in three genotypes ( P 〈0. 01 ). Conclusion There is no significant difference in CRP gene 1059 G/C polymorphism. GG carriers have higher plasma CRP level compared with GC and CC carriers and the myocardial vessel stenosis of GG carries was more severe than that with GC and CC. The prognosis of AMI can be influenced by CRP gene.
