摘  要:目的探讨鼻内镜下低温等离子射频消融术治疗腺样体肥大的可行性和优点。方法对29例并发分泌性中耳炎、慢性鼻窦炎及阻塞性睡眠呼吸暂停低通气综合征的腺样体肥大患儿,在鼻内镜辅助下采取低温等离子射频消融术治疗腺样体肥大,再辅以相应手术和药物治疗后观察其转归情况。结果随访3~12个月,7例(11耳)分泌性中耳炎和18例阻塞性睡眠呼吸暂停低通气综合征均痊愈,3例慢性鼻窦炎痊愈,1例基本痊愈。无腺样体残留及并发症发生。结论鼻内镜下低温等离子射频消融术治疗腺样体肥大,具有直视下进行、病变切除彻底、疗效明显、安全性高、创伤小及并发症少等优点,是一种安全、有效、微创的理想腺样体切除术式。(中国眼耳鼻喉科杂志,2006,6:162~163)Purpose To evaluate the advantages of utilizing endoscope in plasm radiofrequency at low temperature for treatment of adenoid hypertrophy. Methods Twenty-nine cases of hypertrophied adenoid with otitis media with effusion, pediatric chronic sinusitis and obstructive sleep apnea hypopnea syndrome(OSAHS) were treated with nasal endoscopy-assisted radiofrequence-ablation therapy, medicines and surgery was followed. Results All patients had been followed up for 3 to 12 months, 7 patients(11 ear) with otitis media with effusion and 18 OSAHS cured, 3 patients with pediatric chronic sinusitis cured, 1 was almost recovery. No adenoid was residual, no complication ocurred. Conclusion Utilizing endoscope in plasm radiofrequency at low temperature for treatment of adenoid hypertrophy had the advantages of direct and clear view of surgical area that allows the surgeons to remove adenoids accurately and avoid damage of normal nasopharyngeal structures, obvious curative effect, high safety, mini invasive and with few complication.
