右心功能衰竭患者上腔静脉频谱呼吸性变化特点研究

摘　　要：目的 探讨右心功能衰竭患者上腔静脉频谱随呼吸变化的特点。方法 右心功能衰竭患者30例，心功能为纽约心脏病协会心功能分级Ⅲ～Ⅳ级，伴有右心系统的扩大及体循环淤血征象。对照组为以年龄及性别为配对条件的健康人。经右侧锁骨上窝探测上腔静脉，记录平静呼吸时上腔静脉的频谱曲线。观察平静呼吸时上腔静脉频谱形态，选择同一呼吸周期中S波速度最大和最小的频谱曲线，测量选定频谱曲线各波峰值及速度时间积分（VTI），计算其呼吸变化率，采用统计软件SPSS11．0进行配对t检验。结果 健康人上腔静脉频谱S波、D波随呼吸波动明显，吸气相血流速度增大，呼气相血流速度减小，整个呼吸周期S波〉D波。右心功能衰竭时，上腔静脉频谱S波、D波随呼吸的变化减低，同时，频谱形态出现特征性改变。结论 右心功能衰竭时，上腔静脉S波、D波随呼吸的变化率减低，此变化可作为右心功能衰竭的辅助证据。Objective To explore the respiratory changes of spectra of the superior vena cava （SVC） in patients with right heart failure. Methods The spectra of SVC of 30 patients with right heart failure and their paired health people were recorded through right supraclavicular fossa view. The profiles of the spectra of SVC were observed, and the spectra with maximum and minimum velocity of S wave in the same respiratory cycle under spontaneous respiration were selected,and peak velocity and velocity time integral （VTI） of every wave of SVC were measured for statistical analysis. Results In health people,peak velocity and VTI of S wave and D wave of SVC varied prominently in respiratory cycle, and peak velocity of wave S were larger than that of wave D in the whole respiratory cycle. In patients with right heart failure, respiratory changes of S wave and D wave diminished, and the mode of SVC changed. Conclusions The spectra of SVC in patients with right heart failure are abnormal in respiratory cycle,and these characteristics could be used as signs of right heart failure.
