摘　　要：目的:探索改进薄型阴股沟岛状皮瓣转移阴道成形方法及效果。方法:将传统的阴股沟皮瓣改进设计为长蒂薄型阴股沟岛状皮瓣应用于阴道成形。结果:临床应用15例,皮瓣切取最大面积14cm×7cm,13例皮瓣Ⅰ期成活,2例皮瓣尖端部分坏死或表皮糜烂,经换药后痊愈。临床随访6个月～3年,阴道宽松度为2指,深均在10cm以上,无阴道狭窄,皮瓣感觉功能好,外阴形态自然,形成近似正常的阴道外口,已婚者性生活均较满意。结论:改进的长蒂薄型阴股沟岛状皮瓣血运可靠,应用于阴道再造时克服了以往蒂短张力大的不足,移转更方便,避免了阴道成形后皮瓣向外移位的发生,阴道外口形态更满意,保留近似正常的皱襞形态,是阴道再造较理想的方法。Objective To investigate the effect of the modified pudendal-thigh island flap in vaginal reconstruction. Methods Design and apply the longer-pedicled and thin pudendal-thigh island flap in vaginal reconstruction. Results 15 cases were treated with the longer-pedicled and thin pudendal-thigh island flap transfer for vaginoplasty. The size of flap ranged from 7cm×4cm to 14cm×7cm. 13 flaps survived and the other 2 flaps had partial necrosis at the distal partion . Follow-up 6 to 36 months, the flaps have good sensation and spring. The vagina met its physical demands, the vaginal orifice met its physical appearance. Conclusion The modified pudendal-thigh island flap is the ideal method in vaginal reconstruction. It has reliable blood supply and longer pedical , could be transfered conveniently and safely. The reconstructed vagina met its physical demands and physical appearance.
