摘　　要：目的探讨彩色室壁运动分析技术(CK)评价急性心肌梗死(AM I)患者左室整体收缩功能的变化。方法20例AM I患者,分别在择期经皮冠脉介入(PC I)手术前及手术后7天进行CK和声学定量技术(AQ)检查,CK测量心内膜收缩期位移值(ESE),根据CK图像中彩色阶带的宽度将心肌室壁运动异常节段分为2组,AQ测定左室射血分数(EF),并与20例健康人作对照。结果PC I术前,与健康对照组比较,Ⅰ组和Ⅱ组室壁运动异常节段心肌ESE减少(P<0.05),EF降低(P<0.001);PC I术后,Ⅰ组和Ⅱ组室壁运动异常节段心肌的ESE较术前增加(P<0.05),EF增加(P<0.001),且ESE的增加与EF的增加呈正相关(P<0.05)。结论CK可反映AM I患者局部心肌的室壁运动变化和左室整体收缩功能,客观评价PC I手术疗效。Objective To evaluate the changes of systolic function of left ventricular by color kinesis （CK） technique in patients with acute myocardial infarction （AMI）. Method Twenty patients with AMI accepted CK and acoustic quantification（AQ） examinations before and seven days after delayed percutaneous coronary intervention（PCI）. Endocardial systolic excursion（ESE） was assessed by CK, then the patients were divided into two groups according to the width of color excursion in CK imaging. Ejaction fraction（EF） was measured by AQ. The results of ESE and EF in patients were contrasted to those in twenty normal controls. Results Before PCI, the ESE of abnormal regional wall motion segments and EF of left ventricular decreased in group Ⅰ and group Ⅱ compared with those in control group（ PESE 〈 0.05 ,PEF 〈 0. 001 ）. ESE of abnormal segments and EF increased in group Ⅰ and group Ⅱ after PCI（PESE 〈0.05,PEF 〈0. 001 ） ,and the increase of ESE was positively correlated with the increase of EF（P 〈 0, 05 ）. Conclusions CK can reveal the changes of regional wall motion and systolic function of left ventricular in patients with AMI, which provided a direct method for evaluating the efficacy of PCI on patients with AMI.
