摘  要:[目的]研究中性粒细胞即多形核白细胞(polymorphonuclear neutrophil,PMN)凋亡的异常在全身炎性反应综合征(systemic inflammatory response syndrome,SIRS)中的意义。[方法]SIRS患者8例(均为急性胰腺炎患者)为SIRS组,正常人6例为对照组。采用流式细胞仪和TUNEL方法定量检测外周血PMN的凋亡情况,采用ELISA法检测血清IL-6、IL-8的水平。[结果]流式细胞仪检测:SIRS组外周血PMN的细胞凋亡率为(42±8)%,低于对照组的(63±9)%,P<0.05;TUNEL法检测:SIRS组外周血PMN的凋亡指数(apoptosis index,AI)为25±7,低于对照组的37±10,P<0.005。SIRS组外周血血清IL-6水平为(8.2±2.2)pg/mL,高于对照组的(2.1±1.3)pg/mL,P<0.001;SIRS组外周血血清IL-8的水平为(38±12)pg/mL,高于对照组的(12±4)pg/mL,P<0.001。[结论]SIRS患者存在PMN的凋亡延迟,其在SIRS的发生、发展中有着重要的意义。[ Objective] To study the dysregulation of the polymorphonuclear neutrophil (PMN) apoptosis in the systemic inflammatory response syndrome (SIRS), and the clinical significance. [ Methods] Eight acute pancreatitis patients with SIRS, 6 healthy control subjects were observed about the following: apoptosis of PMN in peripheral blood samples analysed by flow cytometry and TUNEL; the values of serum IL-6,IL- 8 detected by ELISA. [Results] Spontaneous apoptosis was significantly delayed in PMN from patients with SIRS compared with controls (42± 8)% vs (63 ± 9)% by flow cytometry, P 〈 0.05 and 25 ± 7 vs 37 ± 10 by TUNEL, P 〈 0.005. The levels of IL- 6 in the SIRS patients was higher than that of controls (8.2 ± 2.2)pg/mL vs (2.1 ± 1.3)pg/mL, P 〈 0.001, the levels of IL - 8 in the SIRS patients was higher than that of controls (38 ± 12)pg/ mL vs (12 ± 4)pg/mL, P 〈 0.001. [ Conclusions] Circulating PMN from acute pancreatitis patients with SIRS show delayed apoptosis, and prolonged PMN survival may contribute to the systemic inflammatory injury characteristic of SIRS.
