系统性红斑狼疮患者EB病毒和人疱疹病毒6型感染的探讨

摘　　要：目的探讨EB病毒(EBV)和人疱疹病毒6型(HHV 6)感染在系统性红斑狼疮(SLE)中的作用。方法采用巢式聚合酶链反应(PCR)检测SLE患者和正常对照者外周血中HHV 6 DNA;应用逆转录PCR(RT-PCR)和Southern杂交技术,检测EBV阳性标本增殖期基因BZLF1、BARF1和BcLF1的表达。结果SLE患者组EBV阳性率高于对照组(P=0),但HHV 6阳性率与对照组差异无显著性(P=0.735 4),SLE患者EBV与HHV 6检出率有明显相关性(P=0.015 9)。32例EBV DNA阳性者中有2例检出BARF1 mRNA,均为活动期,且HHV 6DNA阳性。16例BcLF1 mRNA阳性者中有14例HHV 6 DNA阳性,相关性好。均未检测到BZLF1的表达。结论SLE患者EBV阳性率和BcLF1 mRNA的表达均与HHV 6有明显相关性,SLE患者中可能存在EBV和HHV 6的相互激活作用。Objective To explore the role of Epstein-Barr virus （EBV） and Human herpes virus 6 （ HHV 6） infection in the etiology of systemic lupus erythematosus （SLE）. Methods HHV 6 DNA was tested by nested-polymerase chain reaction（PCR） in peripheral blood of SLE patients and matched normal controls. The expression of EBV lytic genes BZLF1, BARF1 and BcLF1 in the samples of EBV positive patients was tested by RT-PCR and Southern blotting. Results The EBV positive rate of SLE patients was higher than that of normal group （P = 0）, while the difference of the HHV 6 positive rate between the two groups was not significant （P =0. 735 4）. The positivity of EBV and HHV 6 in SLE patients had a significant correlation （P =0. 015 9）. BARF1 mRNA was detected in 2 cases of 32 EBV-positive SLE patients, the positive cases were in SLE active phase, and HHV 6 DNA also positive; Of the 32 SLE patients, 16 exhibited BcLF1 mRNA, 14 of them were HHV 6 positive as well. They had a significant correlation. None was detected the expression of BZLF1. Conclusions In SLE patients the positivities of EBV and BcLF1 mRNA have a significant correlation with HHV 6. EBV and HHV 6 activate each other.
