鼻内镜手术后中药“通窍洗剂”术腔冲洗疗效观察

摘　　要：目的功能性鼻内镜手术(FESS)后“通窍洗剂”术腔冲洗的疗效。方法慢性鼻窦炎、鼻息肉患者60例,随机分为实验组29例和对照组31例。功能性鼻内镜手术后,试验组以“通窍洗剂”冲洗术腔;对照组以生理盐水冲洗术腔。分别于治疗前后行鼻黏膜纤毛运动功能检测,观察症状改善情况及术腔恢复情况。同时,试验组的症状改善情况和术腔黏膜恢复情况也明显优于对照组。结果两组治疗后鼻黏膜纤毛运动功能均有明显改善,实验组传输时间短于对照组(P<0.05)。结论“通窍洗剂”术腔冲洗可以提高功能性鼻内镜手术的疗效,对促进鼻黏膜纤毛运动功能恢复有良好作用。Objective To investigate the therapeutic effect of Tongqiao Lotion（TQL） used postoperatively in a way of nasal irrigation on cases received functional endoscopic surgery. Methods Sixty cases with chronic sinusitis and/or nasal polyps were randomly divided into two groups foUowing functional endoscopic surgery（FESS） to treat the nasosinuous lesions, with 29 cases in treating group （TG） and 31 cases in controlling one（CG）. The cases in TG were postoperatively given TQL to irrigate the operative cavity in nose and/or sinuses, and those in CG were irrigated to the operative cavity with only normal saline postoperatively. Saccharine transit time （STT） assay was carried out among all the cases in this study before and after the therapy to evaluate recovering condition of ciliary function in nasal mucosa, and the improvement of symptoms and mucosal recovery in operative cavity were taken to evaluate the healing course of lesion following FESS. Results While the transit time of saccharine was significantly improved in both groups, STT value was much shorter in TG than that in CG （P 〈 0.01）. Furthermore, the levels of symptoms improving and mucosal recovering in operative cavity were also significantly superior in TG to that in CG following the therapy（P 〈 0.05）. Conclusions Postoperative irrigation to the surgical cavity with TQL on chronic sinusitis and/or nasal polyps further, especially will improve the therapeutic effect of FESS beneficial to the recovery of ciliary function in nasal mucosa.
