不同心脏病充血性心力衰竭病人炎性物质的变化

摘　　要：目的:研究不同心脏病充血性心力衰竭(CHF)病人血清C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)的变化,以探讨CHF的发生、发展机制。方法:50例CHF病人和20例健康对照者为研究对象,利用放射免疫分析法测定血清TNF-α含量,采用散射光比浊法测定血清CRP,用彩色多普勒超声诊断仪测定左室射血分数(LVEF)。结果:(1)CHF病人血清CRP、TNF-α均显著高于对照组(P<0.01),随HF程度加重,血清CRP、TNF-α水平升高,而LVEF值降低,心功能Ⅱ级与Ⅲ级,Ⅲ级与Ⅳ级,Ⅱ级与Ⅳ级组间其含量均有显著性差异(P<0.01)。(2)在冠心病、高心病、扩张型心肌病三组中,血清CRP、TNF-α及LVEF均无显著性差异(P>0.05)。结论:CRP、TNF-α与HF病人的心功能状态密切相关,与其原发病无关,此二者可作为反应HF严重程度的指标。Objective ： To study the changes of serum C - reactive protein （ CRP）, tumor necrosis factor- α（TNF- α） in patients with congestive heart failure（CHF） caused by different heart disease and to discuss the mechanism of CHF. Methods： The levels of serum TNF - α of 50 CHF patients group and 20 normal controls group were determined by radio -immunoassay, while CRP by immunoturbidometry. The left ventricular ejective fraction（LVEF） was examined with echocardiography. Resuits ： （ 1 ） The serum levels of CRP ,TNF - α of CHF group were significently higher than the controls （P 〈0.01 ）. In the CHF group, the serum levels of CRP were elevated according with the worsening of heart failure （ NYHA classification） （ P 〈 0.01 ）. （2） There were no significant difference in serum levels of CRP,TNF - α of the three CHF group with different heart diseases（P 〉 0.05）. Conclusion： It shows the close relationship between cardiac function status and the levels of serum CRP,TNF-α in patients with CHF,but the two were not related to the primary diseases of CHF.
